Accessible Design Consultation Services

The purpose of this service is to provide advice on making homes and workplaces accessible for persons with disabilities.

Case Management

Case Management Services can be used to assist in the management of a file or for specific task assignment on a file. This service can include the following:

  • File Review
  • Gathering of Medical Documentation
  • Meetings with Client and Healthcare Providers Active on File
  • Development and Implementation of a Rehabilitation Plan
  • Arranging Assessments and Rehabilitation Services
  • Return to Work Plan

Chronic Pain Program

The Progressive Goal Attainment Program (PGAP) is a 10 week standardized and cost effective treatment program aimed at reducing disability.  The primary goals of PGAP are to reduce psychosocial barriers to rehabilitation progress, promote re-integration into life-role activities, increase quality of life, and facilitate return-to-work. These goals are achieved through targeted treatment of psychosocial risk factors, structured activity scheduling, graded-activity involvement, goal-setting, problem-solving, and motivational enhancement.  The main components of the program are as follows:

  • Activity Log
  • Activity Scheduling
  • Walking Program
  • Increasing Activity Involvement
  • Overcoming Psychosocial Obstacles to Activity Involvement
  • Evaluating the Benefits of PGAP

An individual being considered for this program will be evaluated prior to commencing the program to ensure they are a good candidate for PGAP. Further information on the PGAP program can be found at https://www.pgapworks.com.

Cognitive Assessments / Cognitive Demands Analysis

Cognitive Assessments are used to provide baseline measures, to assist in defining areas of focus for cognitive rehabilitation, and to evaluate treatment effectiveness. Cognitive Assessments can provide measures for the following:

  • Orientation
  • Attention
  • Memory
  • Executive Functions
  • Perception

A Cognitive Demands Analysis of an individual’s job can also be performed. This is a very helpful tool when determining the return to work potential for an individual with cognitive impairment(s).

Cognitive Functional Capacity Evaluations

A traditional FCE is a systematic evaluation that objectively measures a client’s physical abilities.  The protocols address physical disability and its effect on function, physical durability, and employability.  A Cognitive Functional Capacity Evaluation includes a traditional FCE in addition to taking into account the physical, functional/cognitive, perceptual/sensory, communicative, psycho-emotional and behavioral issues that can accompany ABI. This assessment will accommodate the variety of condition-specific variables typically found in those who have been classified with mild, moderate or severe brain injury.  Testing can be specific to reflect general functional abilities or job specific.

Cognitive Rehabilitation

Cognitive Rehabilitation is a form of rehabilitation used with individuals who have cognitive impairments for various reasons, such as traumatic brain injuries, brain tumors or strokes. Various techniques are used in the rehabilitation process, including the following:

  • Cognitive Training Exercises to Regain Lost Function
  • Training in the Use of Assistive Devices and Cognitive Strategies to Substitute for Lost Function
  • Changing Environmental and Task Demands to Optimize Functional Performance

The Occupational Therapist can also develop return to work plans and strategies considering an individual’s cognitive abilities and the cognitive demands of their job.

Ergonomic Work Site Assessments

An Ergonomic Assessment is an evaluation of an individual’s workstation, work methods, and work environment to determine if there is a good fit between the work and the worker. This assessment includes the following:

  • Assessment of Posture and Body Mechanics
  • Assessment of Workstation and Work Tool Design
  • Assessment of Work Environment
  • Education
  • Recommendations to Increase Worker Comfort, Increase Productivity, and Decrease Risk of Injuries

Functional Abilities Evaluation

The Functional Abilities Evaluation objectively measures the client’s physical abilities. During this evaluation, the client will be asked to perform several tasks and activities which will evaluate his strength, his tolerance, as well as his mobility. This evaluation takes place over two to four hours. This evaluation will help elaborate the rehabilitation plan, and it also monitors the progress during the rehabilitation.

Functional Capacity Evaluations

A Functional Capacity Evaluation (FCE) is a systematic evaluation that objectively measures a client’s physical abilities, including the following:

  • Strength
  • Endurance
  • Range of Motion
  • Workday Tolerance
  • Job Match
  • Includes Measures of Effort and Reliability

Health And Wellness Education

Health and Wellness Education provides information to employees on subjects that are important for a worker’s health and productivity. The goal is injury prevention, health promotion, and productivity. Topics include:

  • Stress Management
  • Ergonomics
  • Safe Work Techniques
  • Chronic Pain Management

In-home Assessments

An In-Home Assessment is completed to identify any safety concerns and/or barriers to optimal functioning in an individual’s home environment. This assessment may include the following:

  • Evaluation of Home Environment
  • Evaluation of the Methods and Tools Used by an Individual in Their Home Environment
  • Education on Topics Such as Posture, Body Mechanics, and Energy Conservation
  • Recommendations for Environmental Modifications, Equipment, Tools, and/or Assistive Devices

Comprehensive Activity of Daily Living Assessments are also available. These assessments identify pre verses post accident functioning and provide recommendations for achieving optimal functioning and compensating for lost function.

Job Site Analysis

A Job Site Analysis is an objective evaluation of the demands of a particular job. This evaluation includes information on the following:

  • Overall Strength Level of Job
  • Job Documentation
  • Essential Job Tasks
  • Mobility/Body Position Requirements
  • Strength Requirements
  • Environment/Ergonomic Risk Factors
  • Sensory/Cognitive Demands
  • Alternate/Modified Work Availability

Pre-employment Functional Assessment

A pre-employment screening assessment consists of determining if the individual has the physical abilities to perform the essential work demands established by the potential employer.   The activities which will be evaluated include strength, tolerance, as well as mobility. This screening process will enable employers to make informed hiring decisions regarding the individual’s physical abilities to perform the job.

Return To Work Programs

A Return to Work program (RTW) is developed by working collaboratively with the client, employer, case manager and health care providers. This service can include the following:

  • On-Site Work Evaluation and Education
  • Recommendations for Work Site Modifications and Adaptations
  • Meetings with Parties Active on File
  • Regular Meetings with Client and Employer
  • Structured Return to Work Plan

Wheelchair And Seating Assessment

The purpose of this assessment is to determine the client’s seating needs and goals. Goals may include improving mobility, improving positioning, preventing pressure sores and maximizing function.   Caregivers are involved with the assessment when appropriate. The assessment will also include discussions around activities of daily living, work or school, leisure activities and the physical environments used.  Once the assessment is complete and recommendations are determined we will work together with the client and a vendor to select the most appropriate equipment to try and provide education.
